Enter the State (MICHIGAN) and the Jurisdiction (BENZIE COUNTY SHERIFF'S OFFICE) and ONE of the following:
- Report number (Most often the Benzie County Sheriff will have given you a business card with the Report number on the back)

- Last name and date of accident
